TO: Chief Medical Officer/Administrative Manager/Medical Director

position. About the Role: As a Nurse Practitioner, you will play a
crucial role in delivering high-quality healthcare to patients
across various demographics. Your primary responsibility will be to
assess, diagnose, and manage patient care in collaboration with
physicians and other healthcare professionals. You will be
instrumental in promoting patient education and preventive care,
ensuring that patients understand their health conditions and
treatment options. Additionally, you will provide comprehensive
care in areas such as primary care, internal medicine, and
obstetrics and gynecology, addressing the diverse needs of the
community. Ultimately, your work will contribute to improved
patient outcomes and enhanced overall health within the population
you serve. Minimum Qualifications: Master's degree in Nursing from
an accredited program. Current Nurse Practitioner certification in
Family Medicine, Internal Medicine, or a related specialty. Valid
state nursing license and certification to practice as a Nurse
Practitioner. Preferred Qualifications: Minimum 1 year experience
as NP Graduated from approved Nurse Practitioner Program. Licensed
as Nurse Practitioner in New York State. Basic CPR (BCLS)
certification required. Specialty Certification Preferred.
Bilingual in English and Spanish or Creole preferred. Experience in
a primary care or outpatient setting. Additional certifications in
specialized areas such as geriatrics or obstetrics and gynecology.
Bilingual skills to effectively communicate with diverse patient
populations. Responsibilities: Conduct thorough clinical
assessments and develop individualized care plans for patients.
Diagnose and treat acute and chronic illnesses, providing
appropriate interventions and follow-up care. Perform routine
examinations, screenings, and preventive health measures to promote
wellness. Educate patients and their families on health management,
disease prevention, and lifestyle modifications. Collaborate with
interdisciplinary teams to ensure comprehensive patient care and
continuity of services. Involved with direct patient physical
assessment, education, and health planning. Assists in the
coordination of services with other disciplines and patient’s
insurance plans. Advocates for patient in arrangement of medically
necessary services. Communicates with community’s resources to
ascertain adequate assistance for patients in need. Refers patients
to specialty services when necessary. Consults with collaborating
physician and other members of the health care team to establish
plan of care. Participate in LEAN and other activities necessary to
implement the multi-disciplinary patient-centered medical home
approach for the care of patients Maintains continuity of care by
regular patient follow-up. Participates in quality assurance
process to assure optimal care delivery. Provides comprehensive
case management services that include emphasis on health promotion
and disease prevention. Monitors patient outcome in accordance with
the practice protocols; Collaborates with staff nurses and nursing
administrators with assessment and clinical decision in patient
care planning. Acts as a resource person for the nursing staff.
Conducts education in service on practice issues. Other duties as
assigned. Skills: The required skills in primary care and clinical
assessments will be utilized daily as you evaluate patient health
and develop treatment plans. Your expertise in internal medicine
and family medicine will guide your decision-making in diagnosing
and managing various health conditions. Skills in obstetrics and
gynecology will enable you to provide comprehensive care to women,
addressing both reproductive health and general wellness. Patient
safety will be a priority in all interactions, ensuring that you
adhere to best practices and protocols. Additionally, your
knowledge in geriatrics will enhance your ability to cater to the
unique needs of older adults, promoting their health and
